GF Strong - School Program
Program Objectives:
The GF Strong School Program provides educational programming and supports and transition planning for adolescent students aged 11 through 18 who are receiving ongoing inpatient or outpatient care with the multi-disciplinary team at GF Strong Rehabilitation Centre. GF Strong Rehabilitation Centre provides rehabilitation services for individuals across British Columbia and the Yukon who require support for symptoms associated with an Acquired Brain Injury. School Program teachers provide instructional and educational support specific to the needs of individuals with these types of injuries.
Referral Process:
Individuals who are receiving care and supports from the GF Strong Rehabilitation Centre care team are eligible to receive educational programming and support with the School Program. Permission for participation in the School Program is obtained through Informed Parent Consent.
Discharge and Transition Planning:
Upon patient discharge from GF Strong Rehabilitation Centre care, the GF School Program teachers ensure a smooth transition back to the school context by preparing Transition Reports and by maintaining ongoing connections with the school team. The GF Strong teachers also provide workshops and resource materials for educators around best practice strategies for supporting students with Acquired Brain Injuries.
